For each query, it checks two things: whether your URL or domain appears in the grounding sources (the actual web references Gemini cited), and whether your brand or domain is mentioned in the response text. A query counts as an "appearance" if either signal is detected. The score maps directly to appearance count: 0 appearances scores 0, 1 appearance scores 4, 2 appearances scores 6, 3 scores 8, and 4 or more scores a perfect 10. A score of 7 or higher is a pass, 4-6 is partial, and 0-3 is a fail.
